You may make an appointment by calling the office at (203) 466-5070 during office hours. When calling to schedule an appointment, remember to have your insurance information available. You will be asked to give a brief description of your medical problem. We accept patients 16 years of age and older. To best care for our patients under the age of 18, an adult or legal guardian must accompany minors. If you need to cancel or reschedule your appointment, please notify us.

Our office requires 48 hours notice for ANY medication refills. Refills are not completed after hours. If you have an emergency, please dial 911.

Medical home services is a team of healthcare professionals who work together to provide a new, expanded type of care to patients under our care   Your appointed Care Team will: * Focus on personalizing your care and treatment * Direct you to community resources that can address any issues/concerns you may have * 24/7 phone support availability (as needed) * We also will provide you same day appointments   We are dedicated to ensure that your healthcare needs are met.

We accept all major insurance plans including Medicaid & Medicare. Payment is expected at the time of service.

If you have an emergency or need to reach a physician after hours, please call the office to access the emergency call system or dial 911.

If your insurance requires a referral, please allow 3-5 business days for this to be completed. Most insurances require Pre-Authorizations for testing and medication. Please allow our office 5-7 business days to complete. If you have any questions please call the office and ask for Laura Maresca

If you have Medicare and live with two or more chronic conditions which could include arthritis, diabetes, depression, and high blood pressure. Chronic Care Management (CCM) services will assist you through these conditions so that you can spend more time living and less time worrying! Services include: Coordination of care between a multitude of providers including hospitals, pharmacies, and more. One or more check-ins per month with a CCM professional, and much more!
