Appointments
You may make an appointment by calling the office at (203) 466-5070 during office hours. When calling to schedule an appointment, remember to have your insurance information available. You will be asked to give a brief description of your medical problem. We accept patients 16 years of age and older. To best care for our patients under the age of 18, an adult or legal guardian must accompany minors.

If you need to cancel or reschedule your appointment, please notify us.
Medication/Refills
Our office requires 48 hours notice for ANY medication refills. Refills are not completed after hours. If you have an emergency, please dial 911.
PCMH (Patient Care Medical Home)
Medical home services is a team of healthcare professionals who work together to provide a new, expanded type of care to patients under our care
 
Your appointed Care Team will:
* Focus on personalizing your care and treatment
* Direct you to community resources that can address any issues/concerns you may have
* 24/7 phone support availability (as needed)
* We also will provide you same day appointments
 
We are dedicated to ensure that your healthcare needs are met.
Insurance & Fees
We accept all major insurance plans including Medicaid & Medicare.

Payment is expected at the time of service.
Emergencies
If you have an emergency or need to reach a physician after hours, please call the office to access the emergency call system or dial 911.
Referrals & Authorizations
If your insurance requires a referral, please allow 3-5 business days for this to be completed. Most insurances require Pre-Authorizations for testing and medication. Please allow our office 5-7 business days to complete. If you have any questions please call the office and ask for Laura Maresca
Chronic Care Management Services
If you have Medicare and live with two or more chronic conditions which could include arthritis, diabetes, depression, and high blood pressure. Chronic Care Management (CCM) services will assist you through these conditions so that you can spend more time living and less time worrying!

Services include: Coordination of care between a multitude of providers including hospitals, pharmacies, and more. One or more check-ins per month with a CCM professional, and much more!

Breast Cancer Screening-The percentage of women 50-74 years of age who had a mammogram to screen for breast cancer improved to (  5.62  % ) 

Breast Cancer Screening Recommendations

The United States Preventive Services Task Force (USPSTF) is an organization made up of doctors and disease experts who look at research on the best way to prevent diseases and make recommendations on how doctors can help patients avoid diseases or find them early.

The USPSTF recommends that women who are 50 to 74 years old and are at average risk for breast cancer get a mammogram every two years. Women who are 40 to 49 years old should talk to their doctor or other health care professional about when to start and how often to get a mammogram. Women should weigh the benefits and risks of screening tests when deciding whether to begin getting mammograms before age 50.

Medication Reconciliation – The percentage of patients who had their medications reconciled at transitions in care improved to (100% )

 

Reviewing the patient’s medication list and making sure we are providing correct medications to the patients when they are seen by another provider or discharged from another facility prevents adverse drug reactions.

Patient experience regarding getting through to the front desk by phone improved to ( 90%   ) based on the results of the patient surveys completed in 2019.  Our goal is to continue to improve access to care to our patients.
